Breast revision surgery is performed to resolve problems related to a primary breast augmentation procedure. Patients may be dealing with implant complications, such as capsular contracture or BII (Breast Implant Illness), or they may be dissatisfied with the shape, size, or projection of their breasts. What is Breast Explant Surgery?

Breast explant surgery is performed to remove breast implants in the safest and most aesthetically viable manner possible. Typically, the surgeon can use the same incisions that were used for the initial breast augmentation procedure, ensuring minimal postoperative scarring. After the implants are removed, old scar tissue in the capsule may also be excised as needed. Dr. Kinal will often perform breast lift surgery or fat grafting after implant removal to ensure the chest appears natural, balanced, and symmetrical.

What Concerns Can Breast Revision Address?

Capsular Contracture

Capsular contracture occurs when excessive scar tissue forms in the pocket around the implants, causing pain and compromising the breast appearance. The condition can be treated with a capsulotomy to expand the capsule and soften the breasts, or it can be addressed with a capsulectomy to remove both the pocket and the implants together, reducing the chance of recurrence.

Double Bubble

Double bubble breast deformity can occur if the implant is poorly placed during breast augmentation, as can happen when a transaxillary or transumbilical incision is chosen. With these approaches, the surgeon typically has a longer distance to contend with between the incisions and the surgical site, which decreases precision. Double bubble can be corrected by repositioning the implants, with or without a breast lift.

Bottoming Out

Bottoming out is an implant malposition complication that occurs when the implants are too heavy and sit too low on the chest wall. There may be loose skin at the bottom of the breasts, and the patient may experience discomfort. Dr. Kinal will correct the condition by repositioning, removing, or replacing the implants. He may also perform a breast lift to combat sagging or hanging skin in the area.

Rippling

Implant rippling can occur regardless of whether the implant is placed above or below the pec muscle. Placing the device above the muscle tends to increase implant visibility, however, as there is no coverage from the muscle. Dr. Kinal can remove or replace the current implant, or he can reposition it beneath the pec muscle. If rippling is caused by over or underfilled saline implants, Dr. Kinal can add or remove the implant solution.

Rupture

Implant rupture is rare, but it may occur due to trauma or impact to the breast, or due to surgical error. With saline implants, it’s easy to tell that an implant has deflated right away. For silicone implants, women must undergo an MRI scan to confirm a rupture or leak has occurred. If the problem is not addressed soon enough, the patient may struggle with discomfort, asymmetry, and excess scar tissue in the capsule. Dr. Kinal will replace or remove the implants.

A Change in Aesthetic Taste

Aesthetic sensibilities tend to change over time, and what was once pleasing may no longer seem appropriate for some women. Some patients may wish to undergo breast implant exchange to change implant sizes, or they may want to undergo breast implant removal to get rid of the devices altogether.
